This Laundry Service Agreement is between ARIAL LAVANDERIA INC. and the COMPANY and/or AUTHORIZED PERSON below. All pricing may be subject to change to reflect industry changes in energy consumption.

ARIAL LAVANDERIA INC agrees to provide the following services:

WASH AND FOLD (included automatically)

* LAUNDRY PICKUP (We pickup your dirty laundry)

* LAUNDRY DELIVERY (We deliver your clean laundry)

DO YOU NEED OTHER SERVICES? (Please describe)

Pickup and/or Delivery has a $5.00 convenience flat fee.

ARIAL LAVANDERIA INC agrees to provide the services listed above at the following intervals:

* How often do you need these services?

Mileage fees will be assessed whenever you request a service dispatch to pick up laundry and laundry services are not needed.

Cancel this agreement any time before your next scheduled pick-up date.

PRICING

Under this Service Agreement, laundry services shall be billed at the following rates:

•Wash & Fold Services shall be billed at $3.25 per lb.

•Delivery Services shall be billed at $0.95 cents per mile.

NOTICES AND ENTIRE AGREEMENT

This Agreement constitutes the entire agreement between the parties with respect to the subject matter of this Agreement and supersedes all other agreements, whether written or oral, between the parties. All notices required or permitted under this Agreement will be provided in writing and delivered to both parties.
